Hospice Registered Nurse (RN Case Manager)

This opportunity is with a financially stable hospice organization dedicated to providing compassionate, high-quality care to patients and families.

Salary and Benefits

• Salary Range: $86,000 – $91,000 (based on hospice experience)
• Company gas card provided for mileage and travel needs
• $36.92 bi-weekly car maintenance stipend

Responsibilities

• Full-time, Monday through Friday schedule
• Based out of the Lawrenceville, GA office
• Manage a caseload of home-based hospice patients
• Coordinate care with an interdisciplinary team
• Perform assessments and create individualized care plans
• Deliver clinical oversight and symptom management
• Educate patients and families on hospice philosophy and services
• Participate in IDG meetings and team care planning
• Provide supervision to LPNs and hospice aides as needed
• Maintain timely and accurate documentation in the HCHB EMR system

Requirements

• Active RN license in Georgia (unencumbered)
• Hospice experience is required (Home Health, ICU, and ER alone will not be considered)
• Current CPR certification
• Valid driver’s license and reliable, insured vehicle
• Proficient in clinical documentation, communication, and assessments
• EMR experience preferred (HCHB a plus)
• Must live locally or be willing to relocate (due to traffic and territory coverage)
